The temple is best known for its grand annual festival, Kadakkal Thiruvathira, celebrated during the Malayalam month of Kumbha (February/March). This ten-day event commemorates the manifestation of Kadakkalamma and attracts thousands of devotees. The festivities begin with the preparation of Pongala, a sweet dish made with rice, jaggery, and coconut, offered to the goddess by thousands of women at dawn. The festival features a vibrant procession called Kuthirayeduppu, followed by rituals like Kuthiyottam and cultural pageantry. The event concludes with the Guruthi ceremony, a powerful and spiritual ritual performed at midnight and continuing until early morning.

The festival has a deep connection with lunar traditions, as it coincides with the Thiruvathira star in the Malayalam calendar, believed to have celestial significance. This connection mirrors the Tamil festival of Arudra Darshan, dedicated to God Nataraja (Shiva), which celebrates the cosmic dance that created the universe. Both festivals highlight the divine cosmic forces and their influence on life and nature.

The temple's rural location amidst agricultural landscapes adds to its charm. Kadakkal is well-known for its spice production and crops like rubber, coconut, tapioca, and pepper.
